Admissions Field RN Hospice

Responsibilities:

Monday through Friday
Admissions of Hospice patients within 48 hours as per physician’s orders
Responsible for providing the initial assessment and admission for patients referred for Hospice Services.
Act as a Liaison with referring institutions for transition of care

Requirements:

A Registered Professional Nurse with current License
Hospice agency experience required
Knowledge of hospice regulations and qualifications for hospice admission.
Associates Degree; Bachelor’s degree preferred
