RN Team Leader Hospice

Responsibilities:

Hybrid role of management and field-based RN
Management of an interdisciplinary team of caregivers
Education and Training of clinicians
Assist clinicians in developing a Plan of Care
Carry an assigned partial caseload of Hospice patients

Requirements:

RN License Massachusetts
valid Drivers License and willingness to travel to patients’ homes
COVID Vaccination required
Hospice or home health experience
Supervisory experience preferred
OASIS experience preferred
Compassion and integrity
