The Pediatric RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end of life care in the community setting. The RN Case Manager is responsible for promoting the Hospice philosophy. This individual facilitates the development of a collaborative interdisciplinary plan and ensures the overall coordination, collaboration and medical management of hospice services for patients and their significant others.

Must have pediatric hospice experience.  $12,500 Sign-on for qualified Pediatric Hospice RN’s.
