Occupational Therapist Home Health- Nassau County

Responsibilities:

Initial Evaluation pf patient’s functional status in a home setting
Development and implementation of a plan of care
Ongoing one-on-one treatment in the home
Educate patients and caregivers on the plan of care and individualized treatment plan
Maintains and submits all clinical documentation in compliance with Agency standards

Requirements:

Occupational Therapy License New York State
Minimum one year clinical practice. Home Care preferred
Valid NYS Drivers License and reliable transportation
Travel throughout the Borough of Queens
