We are seeking a Licensed Social Worker to join our Home Health team on a PRN basis near Meridian, Idaho. The ideal candidate will have at least one year of experience working in Home Health, as well as a valid licensure as a social worker in the state of Idaho. This position will be responsible for providing social work services to patients in their homes, working closely with our interdisciplinary team to ensure that patients receive the highest level of care possible.

Key Responsibilities:
 
• Conducting in-home assessments of patient needs, including psychosocial assessments, and identifying any issues that may hinder patient progress.
 
• Developing and implementing care plans that address the social, emotional, and psychological needs of patients.
 
• Providing counseling and support to patients and their families, including grief support and end-of-life care.
 
• Collaborating with other members of the interdisciplinary team, including nurses, therapists, and physicians, to ensure that patients receive comprehensive care.
 
• Documenting all patient interactions in accordance with state and federal regulations.
 
• Maintaining communication with the patient’s primary care provider, as well as other healthcare providers involved in their care.
 
Qualifications:
 
• A valid license as a social worker in the state.
 
• At least one year of experience working in Home Health.
 
• Knowledge of federal and state regulations related to Home Health.
 
• Excellent communication and interpersonal skills.
 
• Ability to work independently and as part of a team.
 
• Ability to assess patient needs and develop effective care plans.
 
• Strong organizational and time-management skills.
 
• Ability to work flexible hours, including evenings and weekends.
