Hospice Weekend RN

Responsibilities:

Fairfield and New Haven counties
Completes initial comprehensive and ongoing assessment of patient and family to determine Hospice needs
Admissions of patient for services and development of the initial patient/family Plan of Care
Coordination of services with the patient, family, and hospice interdisciplinary team members

Requirements:

Valid Registered Nurse (RN) license
RN Diploma, Associates or B.S. degree in nursing
Minimum 1 year of home care or hospice
Valid Driver’s License, auto insurance and reliable transportation
Excellent observation, problem solving and communication skills
Compassion and integrity
