Responsibilities:

Assess patient to determine skilled home health care needs
Develop a Plan of Care to address patient’s needs
Re-assessments and changes to Plan of Care as necessary
Supervision of caregiver staff assigned to the patient
Patient and family education for safety, best care outcomes, and to prevent hospital re-admissions
Develop discharge planning needs when goals have been met

Requirements:

Valid & current RN license
Skilled Home Health and OASIS experience preferred
Acute Care experience preferred
Ability to learn EMR software
